The description should be short and clear and include the employer’s contact information. The address should be the employer’s place of business, not a post office box.

In addition to the name and company, the reference should be listed separately. For more detail, the person’s contact information should include an email address and phone number. The reference should not have a personal or private email address and you should not include their personal details on your CV.
